*11 *Quart container of chocolate milk was found offered for sale in self-service refrigeration, past the ‘sell-by’ date (11-16-24) listed on the container.
Affected product was removed from the sales floor during inspection.
 Corrected On-Site.  New Violation.
*14 *The soda dispensing nozzles and ice chute on the beverage dispenser are unclean with a slimy, bacteria like (dispensers) substance and a mold-like (ice chute) substance.
Thoroughly clean affected food contact surfaces and sanitize prior to next use.
Beverage dispenser was placed out-of-use during inspection for cleaning.

*22 *Pizza slices and chicken wings in the hot holding cabinet are not labeled to indicate a time by which they are to be sold, or discarded.
If time, rather than temperature, is used as the public health control the food must be marked or otherwise identified to indicate the time that is 4 hours past the point in time when the food is removed from temperature control.
Per facility staff, one-hour time control is used for pizza and bone-in and boneless wings.

34 No thermometer is present in the countertop hot holding case at the front register.
